New Graduate Student ARRIVAL CHECKLIST

Welcome to IBP! As a new graduate student, the following checklist has been designed to guide you through the employment process and help you complete the necessary tasks and paperwork when joining our campus.
CHECK-IN WITH YOUR DEPARTMENT ADMINISTRATOR (DA). The Department Administrator (DA) should cover the following with you:

Payroll Authorization: A Personal Data Authorization Form must be filled out and turned in to Human Resources for you to be put on payroll at the U of MN. Please check with your Department Administrator as to how this is submitted in your department.

Keys and Building Access: A Key Request Form must be filled out and sent to AHCRBMS in order for you to obtain your key(s). Please check with your Department Administrator who will guide you in making this request and who serves as the approving party for your access to buildings and labs. You will need to provide your Employee ID number for this process.

Bring H1B, passport, and letter of appointment.
COMPLETE REQUIRED FORMS AT HUMAN RESOURCES OFFICE.

All of your records and contact information will be handled through the Office of Human Resources. You will be asked to fill out an I9 and federal and state tax forms, as well as set up direct deposit, parking etc. Your HR contact for IBP is Molly Rochford (rochf003@umn.edu or via phone: 612-625-3687) in Jackson Hall 6-120.

OBTAIN UNIVERSITY PHOTO ID CARD.

Bring a copy of your appointment letter and two forms of ID (passport, driver’s license). Be prepared to have your photo taken in Coffman Union for your U of MN ID:

OBTAIN PARKING TAG, KEYS AND BUILDING ACCESS
COMPLETE MANDATORY SAFETY TRAINING

You must complete mandatory safety training before gaining access to the laboratory. Go to: www.dehs.umn.edu .

Click on “Training”

Click on “Radioactive Materials” and “Radiation Safety Orientation”

Once you have completed the training, send an email confirmation to ahcrbms@umn.edu and present your U card at the Information Desk on the 3rd floor of NHH for processing.

Access to the lab will be granted within 48 hours
ATTEND HUMAN RESOURCES BENEFITS ORIENTATION

Contact Molly Rochford, our HR Specialist, with questions about benefits. Benefits orientations are held periodically throughout the academic year. You can find one that is convenient for your schedule by going to: http://www1.umn.edu/ohr/benefits/events/index.html#workshop. The workshops cover enrollment in medical, dental, vision, prescription plans, as well as retirement plan enrollment and automatic salary deductions.
